Important Information about 2022 Employee Benefits Open Enrollment

This year we are doing a passive enrollment which means if you do nothing by the end of the business day on 11/22, your current plans will carry forward to 2022. However, since we are changing to a new health insurance carrier and there are plan design changes, it is important to review the attached Medical Plan Comparison to ensure the plan you will be mapped to will meet your needs for 2022. The dental and vision plans remain the same with Lincoln.

Please note:

Page 6 of the guide lists examples of no-cost services that apply to any of the three Gravie Comfort (PPO) plans. As in the past, all four plans have preventative services covered at 100%. The HSA plan participants can take advantage of their virtual visits through MDLive, at discounted rates before meeting the deductible.     Dermatology: $59

    Mental Health visits range : $45 - $250. 


Triada Enrollment 

During the week of 11/29/21, you will receive an email from Triada Health with login information to complete your 2022 enrollment for:

  • GAP+
  • HIP+
  • Short Term Disability
  • Accident
  • Critical Illness

Triada requires you to log in and make elections annually. If you do nothing, you will not have any Triada plans for 2022.
