To submit a Flexible Spending Account (FSA) Claim:
Please follow these steps for each receipt:

1. Log in to Paylocity and navigate to:
   - Self-Service Portal
   - Benefits
   - Spending Accounts
   - File a Claim

2. Enter the following information:
   - Start and End dates of service
   - Claim Amount
   - Provider Name
   - Category
   - Type
   - Recipient/Patient
   - Description (if necessary)
   - Mileage (if applicable)

3. Drag and drop or upload the corresponding file from your computer that supports the claim.
