IMPORTANT INSTRUCTIONS - If you are injured on the job, follow the steps below to report a claim and get medical attention, if needed:
If the incident is an emergency, a coworker should call 9-1-1 immediately.
If not, please follow these instructions:
1. Notify your supervisor immediately
Late reporting of claims may jeopardize the acceptance of the claim by the insurance carrier
2. Complete the Employee's Report of Injury form
3. If treatment is needed, email email@example.com with ! high importance
